Greetings. I took at look at your model, and the animations were still intact.
Moving things around within FPSC...floors, ceilings, walls...sometimes mathematics get a little off.
After examining your model, the first thought that came to mind was that the conditions weren't being met for the .fpi to activate the animations.
I adjusted the plrdistwithin value of the .fpi file, and gave it a test. The hud prompt and animations are now working fine.

You will need to make animation adjustments to the distance that the door travels, as it currently does not travel far enough to allow the player into a passage.
I've attached the working folder that I used to test your door. Place
Floor Door anywhere you choose, and place
FloorDoor into
scriptbank\user. You'll be able to take it for a test drive.
