Doors: When you are in the segment mode and you would like to put a door down, just press "r" and it will rotate the segment or in other words the Door.
Entities (Objects): When you are in the entity mode, just as the segment rotate the object by pressing "r" on your keyboard.

Well, Fps Creator is using Models in the .x file. If character shop can export a character in a .x file you can import it into Fps Creator.

Well, if you mean like making your own walls and floors. There are a lot of programs out there. Search around these boards. You might get some.
