Ahhhh... Yeah.

Darkness, skeletons, blood... not scary.

Good lighting - Make the player see what you want him to see, and leave some darkness around, but not pitch black.

Sounds - Spooky background music and sudden triggered sounds.

Objects - Characters and other objects that seem to appear suddenly and from nowhere.

Confusion - Keeping the player lost or disoriented with paths that go nowhere or lead back onto themselves.

I'm sure others can think of more...

Best.

I'm sorry, my answers are limited. You must ask the right question.

Atmosphere.

Forget lots of guns and enemies, just make sure there is tension in the air and keep the player guessing. Good use of sound is really important for a horror game. You've got to get the player immersed if you want them to truly realise your knightmare.

I'm more into action with an element of atmosphere as I found out after taking Silent Hill 2 back to the shop 4 days after purchase a few years back. Man that game freaked me out. Might be a good referece material for you if you've not already played it and want to go for more of an attack on the senses than the trigger finger.

Depends how twisted you are I guess and how much you want the player to suffer. Doom 3 was pretty intense but F.E.A.R was about the right balance for me. I guess playing these games in the middle of the night with the lights out and volume up might be where I'm going wrong.
