get an editor like iconed to make them and a package like adobe photoshop or paintshop pro.

Alternativly you could get a cheap grahpics program and a free icon converter off the net, but you would have to search around unless someone else can mention one.

Use Pixel ToolBox to do that kind of thing. you can also make mouse cursors and patterns.
You can find it at download.com in the desktop section.
