The most common is with Milkshape 3d (search) and textures are either made with Photoshop or Gimp (search)
The easiest way to get them into FPSC is by Entity Workshop 2 (search) where you can scale your model etc and then simply click save model and you can find it in FPSC.

Or if you want a free route use Wings 3D. It's VERY easy to use. But it doesn't support animation =(
