I have had this error several time when using custom media... it is usually cause by problem in the mesh of the entity.. I believe it is related to instancing in some way...

I had entities that were using the same mesh using 2 different mesh names and it would always give the error when it would make the bin and dbo file for the second entity on the level.

Another cause for this error I have found is not saving before testing.. it seem to be random though in this case... To get around it always save then test... even if you just opened the map... save before you test it...
