the DirectX JT exporter is one of the options when exporting a file from milkshape.
BTW puppy, I noticed you signed up for my forums, but never activated the account. Did you forget, or were you having trouble activating?

EDIT: oops, didn't see your attached pic. You obviously already have milkshape. When you go to export your model, just scroll down to DirectX JT. It should be down near the very bottom.
