If you require a custom model you are willing to contract (royalty-free) then I can do it for ya.
Charges depend on the complexity of the work required.
You can choose the cost of the work.
I charge a set £15 per model, plus £5 per texture skin. The model is exported in DirectX format only, with animations as seperate files to 'append'. For this price you get a Current LOD Mesh Pack, but is subject to Mass Production Limitations.
If you would like animations then you can add £5 per animation, obviously single animations that are split into several count as one. Such-as Walking (Start, Loop, Stop)
This means that if you charge then you can use it royalty-free for up to 500 sold copies of your software. This can be either physical or downloadable formats.
You can extend this for every 1,000 additional copies for an additional £10 each.
Alternatively you can choose to have the unlimited Royalty-Free service. You would be charged £15 per hour of work. This includes everything you have asked for, not just the mesh.
Either choice requires for credit to be given somewhere in your manual / software for the work provided.
The original Maya Scene (binary format) and Photoshop Texture Map (PSD 8 compatible format) is provided for any additional in-house editing you would wish to do.
You will need to specify the polygon counts you are looking for, generally speaking I provide ranges that I consider to be Low, Medium, High and Detailed by an adveraged standard.

Classic - Ranges

The labels indicate generation.
Exporting to a format like 3DS is a problem, as it doesn't support Skinned Mesh Animation. This means that the models have to be created and animated as Template Objects.
It is also a problem given that 3DS is a very problematic format to use in engines. Many incompatibilities may show up. Just a warning about the format, exporting to it would be no problem though.
Also while descriptions are good, visual references can help greatly to getting the exact look you want. Any images that could help the process would be helpful.

It might be cheaper to wait and purchase the standard character model, then contract a unique skin.
